Japanese Customs Declaration Form

I just returned to Japan from my trip to Philly (i.e Philadelphia) and while completing the Japanese customs declaration, I noticed a number of mistakes. Here are the excerpts from the form, try and correct it yourself and then below for the correct answers:
- Any items you have been requested from someone eles to bring into Japan
- …please write your belongings in “Description of Personal Effects” of side B.
- “REPORT OF CARRYING OF MEANS OF PAYMENT”
- If the total market value of each personal items dose not exceed ¥10,000, you need not write it to this Declaration Form.
- There is no duty-free allowances for alcoholic beverages and tobacco products for those under 20 years old.
- As for children under 6 years old, duty-free allowances is limited to the articles owned for their personal use.
Here are my corrections:
